Yes, as the steel gears will shave themselves as they bed in. Little point on the sump, as you shouldn't have great chunks of steel coming off your engine.
Shouldn't do. But might. I fitted one to my last car after an engine rebuild and it looked similar to the pic above after 2k miles.
I'd suggest something horrible was happening inside. I'd expect to see soft material in the oil, but not lumps of steel...
Yeh, it was like iron filings. Not lumps by any means. Felt happier that it came out of the engine though!
